If cancellation is necessary we require 48 hours notice. In case you are unable to provide 48 hour notice cancellation fees will apply as follows:
Initial Visit $75
Follow-up Visits $50
You will be invoiced automatically.
Medical papers, insurance forms and other requests not covered by OHIP will be charged to the patient depending on each situation and length of time it takes to complete. Rates will be provided upon request.
